Study Title: Neuroarts Academic Network Survey

Introduction and Contact Information: You are asked to take part in a research study. Participation is voluntary. The lead researcher is Susan Magsamen, Executive Director of the International Arts and Mind Lab at Johns Hopkins School of Medicine. Please read this form and feel free to ask questions. Description of the Project: You are being asked to participate in a study examining how educational institutions train, support, and prepare individuals across multiple disciplines to apply neuroarts-related knowledge in the workforce. Your responses will help identify gaps, opportunities, and strategies for strengthening education and career pathways, developing courses and curricular offerings, and shaping institutional infrastructure. Your participation in this study will take approximately 10-15 minutes. You must be at least 18 years old (or of legal adult age in your location) to take part.

Risks: There are minimal risks involved. Your survey data will be confidential in that they will not be linked to any of your identifying information (like your name or email). You can stop at any time.

Benefits: There are no direct benefits associated with participating in this study beyond contributing to the emerging field of Neuroarts.

Compensation: There is no offered compensation for participating in this research.

Confidentiality: Your part in this research is confidential. If results from this study were to be published or presented at research conferences and/or shared with other researchers through an online data repository, the information gathered for this project will not be published or presented in a way that would allow anyone to identify you.

At no point will anyone attempt to align your survey responses with your personal information. Any data gathered for this project will be password protected and only the research team will have access to the data. 

Voluntary Participation: The decision whether or not to take part in this research study is voluntary. If you do decide to take part in this study, you may end your participation at any time without consequence. If you wish to end your participation, you should exit the survey and close your web browser. Whatever you decide will in no way penalize you or involve a loss of benefits to which you are otherwise entitled.

Questions: You have the right to ask questions about this research before you agree to be in this study and at any time during the study. If you have any questions or concerns about your rights as a research participant, please contact a representative of the Institutional Review Board (IRB), at Johns Hopkins University School of Medicine which oversees research involving human participants.
